Ganciclovir 5 mg/kg/12h Perf. 1 h
Gentamycine 6 mg/kg/j 1x/j Perf 30-60 min arrêt après 3 jours
Imipenem >1 mois 25mg/kg IV/6h max 1g/dose (ajust. fn rénale)
Meropenem ≥3 mois: 20mg/kg IV/8h (Infections SNC: 40mg/kg IV/8h); ≥3 mois : max 1g/dose, Inf SNC: max 2g/dose (ajust. fn rénale)
Métronidazole 10mg/kg/8h PO/IV max 1.5 g/j
Piperacillin/Tazobactam <9 mois: 80mg/kg IV/8h; ≥9 mois et ≤40kg: 100mg/kg IV/ 8h; >40kg: 3g IV/6h ou 4g IV/ 6-8h dose quot. max 16g/jour (ajust. fn rénale)
Vancomycine 15-20mg/kg/dose/8h max 2g/j, jusqu’à 4g/j si infection SNC
Voriconazole 6mg/kg/12h IV ou PO à J1 puis 4mg/kg/12h.
